HOW TO CUSTOMIZE A QUESTIONNAIRE
We offer the ability to create a questionnaire for you using a template that you provide us. This process falls under the concept of professional services for which there is a fee associated. Once you provide us with a template, the development team will review said template and provide a quote as to how much creating this template would cost. The template can include any questions that you need answered in whatever order you need them answered in. Any data that cannot populate a field in the client profile will be created as a custom field. If you prefer to create your own intake forms, this can be done via the administrative tools, here is our article on that process. The last option is customizing the pages of the questionnaire that appear for a given case process, these steps are outlined below.
Via the administrative tools, one can customize the different pages of the questionnaire that can be sent for a given case process.
- Navigate to the Administrative Tools.
- Click on the Case Processes icon on the Home page.
- Choose a case process that you'd like to update the questionnaire list for, and click the button under the Questionnaires header.
- From this screen, you can delete pages that are already listed as part of the case process questionnaire by clicking the . You can also add more pages to the process questionnaire by clicking the Add Process Questionnaire(s) button.
- Once you click the Add Process Questionnaire(s) button, you will be brought to a page which allows you to check off the additional pages of the questionnaire that you wish to add. When ready, click Add Questionnaires.
